Terms and Conditions that apply to the My Back Relief Clinic No-Gaps Initial Consultation:
- Offer only available for Initial first time patients to My Back Relief Clinic. No treatment is offered in this offer
- Offer only available to patients who have private health insurance with an Australian health fund who are able to claim their available health fund rebates for Chiropractic cover at My Back Relief Clinic
- Must provide a physical valid health fund card to swipe on a HICAPS machine
- Health Fund annual benefit service expiration has not been exceeded.
- Offer covers potential initial consultation Item Numbers 1005 and 1112
- Complete insurance criteria requirements are satisfied. Health fund limits and rules apply. Patient must confirm their eligibility with their health fund provider prior to appointment to claim rebates.
*Please note we can not back claim, claims must be done on the same day of the appointment.
